Dear State PATH Contacts and PATH Providers: As a reminder, the proposed PATH annual report was released for public comment in a Federal Register Notice (FRN) on May 18, 2015. The FRN provides a summary of the proposed changes to the report, and the full proposed PATH Annual Report data elements and guidance is available here . Please note that the manual is not final and PATH providers should continue to collect data according to the current PATH Annual Report form and PATH Annual Report Provider Guide . The proposed data elements and guidance are subject to change as a result of the public review and comment period.
